If you qualify, you’ll get a personal care manager to help you schedule appointments (like for regular checkups, eye exams, or mental health counseling), find safe housing, get job training, and more. You’ve got a lot going on—this takes one thing off your plate.

2 - Free help will come to you

Your care manager can meet you wherever is best for you, like a doctor’s office, coffee shop, or wherever you’re living.

3 - Things don’t change overnight. Help is there for you along the way.

If you qualify, you can get free support for 12 months or more (if you’re under 26). Your care manager works with you that whole time to keep your health on track after foster care. The best way to start is to call your Medi-Cal health insurance plan.
